*The School ID Number box is optional. If your Athletic Director/Advisor advised you to use an ID number, enter the ID number in the box. By entering the school's ID number, your A.D. will be able to see that you completed this form in their A.D. portal. Some Athletic Directors do not require completion of this form. If that's the case, simply leave the third box blank.

The results page can be printed off and placed in a binder or folder for future use.
